Medical History

Knowing your complete medical history will make your procedure as safe as is possible. We offer several methods of providing your medical history so you can pick the method that suits you best. Only one of the following is needed.

• Method 1: Print the form and complete by hand: Method 1
• Method 2: Fill the form out on line, then print the completed form: Method 2
• Method 3: Download the form in MS Word format, complete and print or print and complete: Method 3

Whichever method you choose, don't forget to bring your completed medical history form to your consultation visit.
